Newer aspects in structure of dermatics

Súhrn: Trendy spojené s vývojom dermatík a kozmetík sú
poznamenané všeobecnými tendenciami o určitú ekologizáciu prípravkov. V záujme
ochrany a udržania fungujúcej kožnej bariéry, resp. jej úspešnej regenerácie,
sa pozornosť odborníkov pri projektovaní prípravkov sústreďuje najmä na výber
biokompatibilnejších východiskových surovín a menej agresívnych pomocných
látok.
Do popredia sa dostávajú aj snahy o multifunkčnosť prípravkov, najmä v
skupine kozmetík (šampón + kondicionér, pleťové krémy + UV filtre, a
pod.).
Tendencie súvisiace s efektívnejším ovplyvnením terapeutického účinku
liečiv aplikovaných na kožu sa premietajú do hľadania a formulácie nových
štruktúr dermatík a nových výrobných postupov. Tieto majú za cieľ
modifikovať mieru a rýchlosť uvoľňovania liečiv z vehikúl, hĺbku penetrácie
liečiva, cielený transport a dobu pretrvávania liečiva na mieste očakávaného
účinku.
Reklama, ktorá tieto nové typy a štruktúry sprevádza, je veľmi často
neobjektívna.

Summary: Trends in
development of topical preperations for dermatology are influenced by ecology.
In the interest of protection and maintance of skin barier, resp. its
regeneration, the attention of experts in plan of new preparations is focused on
the choice of more biocompatibile raw material as well as less agresive
ingredients. Very interesting is effort for more effects in one preparation
(shampoo + conditioner, moisturizer cream + UV filter).
The trends to be related with more effective influence of therapeutical effect
of drugs apply on the skin project on to search of formulation of new structure
of topical drugs. The aim of these trends is to modify the speed of active
substance release from the vehicle, deph of penetration to the skin, transport
as well as the time of drug remaining in the place of action.
Advertisement about this new structure mainly in cosmetics is very offen not
objective.

Biosynthetic skin substitutes for controled skin reconstruction

Súhrn: V klinickej praxi sa čoraz viac stretávame s požiadavkami a
potrebou syntetických bioresorbovateľných implantátov, používaných pri účinnejšej
reparácii rán. V súvislosti s tým možno badať pokrok najmä pri liečbe
popálenín, kde požiadavky na dokonalejšie, sofistikované kožné náhrady sú
formulované jednoznačne smerom k rýchle sa rozvíjajúcemu tkanivovému inžinierstvu.
Očakáva sa, že tkanivové inžinierstvo kožných náhrad poskytne perspektívny
zdroj pre progresívne terapeutické postupy pri liečbe akútnych a chronických
rán. Najbežnejšie stratégie tkanivového inžinierstva zahŕňajú použitie
izolovaných buniek alebo in vitro kultivovaných buniek vysiatych na polymérnu
bioresorbovateľnú matricu s trojrozmernou štruktúrou, s vhodnými biologickými
a mechanickými vlastnosťami. V tkanivovom inžinierstve sa používajú prírodné
alebo syntetické biodegradovateľné polyméry, alebo ich kombinácie, ktoré
vytvárajú matrice pre kultiváciu buniek. Náhrady kože pozostávajúce z
kultivovaných buniek a bioresorbovateľných polymérov môžu slúžiť aj ako
alternatívne materiály pre štúdium biológie a patológie kože, hojenie rán,
ale aj na testovanie bezpečnosti spotrebiteľských výrobkov. Každá z týchto
perspektívnych aplikácií si vyžaduje špeciálne hodnotenie týchto náhrad.
Článok sa orientuje na využitie kožných náhrad pre liečenie a prekrytie
kožných defektov a v krátkosti predstavuje membránu Coladerm vytvorenú
komplexáciou hyaluronanu a enzymaticky opracovaného kolagénu. Komplex je
chemicky sieťovaný dialdehydovými derivátmi škrobu. Táto membrána je
mechanicky stabilná, vhodná pre sutúru, aplikovateľná na prekrytie rany a
hojenie chronických, akútnych a povrchových chirurgických a hlbokých rán.

Summary: The clinical requirement for artificial graft materials to
promote effective wound repair is large. Treatment of chronic or extensive
wounds, and progress in burn care is dependent much on a suitable sophisticated
skin substitute. With regard to woundcare, the desirable final goal is that the
engineered tissue becomes integrated within the patient, affording a potentially
permanent and specific cure. The most common tissue engineering strategies
involve the use of isolated cells or cell substitutes, tissue-inducing
substances, and cells seeded on or within polymer matrices with a
three-dimensional architecture as well as appropriate biological and mechanical
properties. Biodegradable polymers – natural and/or synthetic are used as a
matrix for living cells. Especially, skin substitutes composed of cultured cells
and biopolymers provide alternative materials for study of skin biology and
pathology, treatment of skin wounds, safety testing of consumer products, and
therapeutic delivery of gene products. Each of these prospective applications
may have distinct requirements for validation of skin substitutes. However, this
paper is focused on considerations with respect to treatment and closure of skin
wounds and presents shortly the Coladerm membrane based on a complex of
hyaluronan and enzymatically treated collagen. The complex is crosslinked with
starch dialdehyde derivatives. This hybrid membrane is mechanically stable and
suitable for suture, ready-to-use for covering and healing of chronic, acute,
and surgical superficial, partial or full-thickness wounds in humans.

Global harmonisation of cosmetic regulation and INCI labelling of ingredients

Súhrn: Požiadavka harmonizácie slovenskej legislatívy s právom EÚ
znamená potrebu aproximácie dokumentov tiež v oblasti kozmetických
prostriedkov. Platnou smernicou v krajinách Európskej únie je 6. doplnok EU
Cosmetic Directive. Slovenská republika, rovnako ako ostatné prizvané štáty
do EÚ, túto smernicu akceptovala a v súčasnosti ukončuje jej plnú
implementáciu do legislatívnych predpisov.
Vo svete prebiehajú negociačné stretnutia o nutnosti globálnej harmonizácie
kozmetickej legislatívy. Kľúčovými potrebami sú: zjednotenie definície
kozmetických prostriedkov, bezpečnosť produktov, trhová kontrola, jednotné
označovanie produktu a označovanie zložiek podľa INCI.
Medzinárodná nomenklatúra kozmetických zložiek (INCI) je povinná pre členské
štáty EÚ a akceptovaná v ďalších krajinách sveta. Predstavuje technický
kód pre identifikáciu látok použitých v kozmetickom prostriedku jednoduchším
spôsobom než je medzinárodné chemické názvoslovie. Povinné jednotné označovanie
zložiek na obale kozmetického prostriedku je nesporným prínosom pre
spotrebiteľov s citlivou pokožkou, alergikov, ale aj pre ošetrujúcich
dermatológov.
Práca poskytuje prehľad najdôležitejších olejov, tukov, voskov a rastlín
používaných v kozmetike a ich označenie podľa INCI.

Summary: The requirement of harmonization of the Slovak legislation to
EC laws means the necessity of approximation of the legislation relating to
cosmetic products, too. The current status in EU member states is the 6th
Amendment of EU cosmetic Directive. Slovak Republic as well as the other invited
countries to EU accepted this EC cosmetic law. It is finished the full
implementation of this status in the Slovak cosmetic legislation in the present
time.
There are held the negotiation meetings about the necessity of the global
harmonization of cosmetic regulation in the world. The key pillars of
harmonization are: unify of cosmetic product definition, product safety,
in-market control, the unify labelling of product and INCI labelling of
ingredients.
International Nomenclature Cosmetic Ingredients (INCI) is an obligatory for EU
countries but accepted in many countries around the world. It represents a
technical code for identification of ingredients used in cosmetic products the
more simple way than is possible by the international chemical nomenclature. The
obligatory unify cosmetic ingredients labelling is the contribution not only for
consumer or for person with a sensitive skin but for dermatologists, too.
The paper provides an outline of the most important oils, fats, waxes and
botanicals used in cosmetics and their INCI labelling.

Present state in adjuvant immunotherapy of melanoma

Súhrn: Napriek pokroku v klasifikácii a chirurgickej liečbe, u
pacientov s vysokorizikovým melanómom je pravdepodobnosť rekurencie choroby
počas 5 rokov od 55 % do 80 % a 5-ročné prežívanie len od 25% do 70 %. Pre
túto skupinu pacientov sa účinná adjuvantná liečba javí veľmi potrebnou.
Nízko dávkovaný interferón v adjuvantnej liečbe nedokázal ovplyvniť
celkové prežívanie a výsledky, čo sa týka bezrelapsového prežívania, sú
u rôznych skupín pacientov nesúrodé. Vysokodávkovaný interferón v
adjuvantnej liečbe zlepšuje bezrelapsové aj celkové prežívanie v štúdii
E1684 a Intergroup E1694 v štúdii. Vysokodávkovaný interferón zapríčiňuje
signifikantnú morbiditu, ale je prínosný, pokiaľ ide o kvalitu získaných
rokov života. Adjuvantná liečba vysokodávkovaným interferónom sa dnes považuje
za štandardnú terapiu pre pacientov s vysokorizikovým melanómom, u ktorých
je predpoklad, že takúto terapiu budú tolerovať. Táto liečba by v budúcnosti
mala slúžiť ako referenčný režim pri skúšaní iných možností
adjuvantnej liečby u týchto pacientov.

Summary: Despite advances in the staging and surgical therapy of
melanoma, patients with high risk melanoma still have 5-years recurrence rates
of 55 % to 80 % and 5-year survival rates as low as 25 % to 70 %. Effective
adjuvant therapy is needed for this patient population. Low-dose adjuvant
interferon regimens have not affected overall survival and have had an
inconsistent effect on disease free survival across different stage groupings.
High-dose adjuvant interferon improved disease free and overall survival in the
E1684 and Intergroup E1694 trials. High-dose interferon regimens cause
significant morbidity, but quality-adjusted years of life are greater with this
therapy. Adjuvant high-dose interferon should be considered standard therapy for
all high-risk melanoma patients expected to be able to tolerate the interferon
and treated off protocol. In addition, this regimen should serve as the active
control in the future trials of alternative adjuvant therapies for these
patients.
